How To Enroll:
Enroll Online or Stop in the Child Support Office to complete an Enrollment Form
Documentation that you will need:
Birth Certificate for each child
Social Security Card for each child
Applicant’s Driver’s License or State Issued Photo I.D.
Copies of any Court documents regarding divorce, paternity, guardianship, child support
Medical card for each child
Paternity affidavit for each child if available
